ACCS Request for Conciliation Form

The Request for Conciliation Form is available for download in both MSWord format and pdf format. The MSWord version of the form allows you to complete all details on your computer prior to printing. The pdf version of the form must be printed and completed manually.

To request WorkCover Assist help you with your dispute, on the ACCS Request for Conciliation Form where it asks:

Have you arranged for your union or another person to assist you at conciliation?

1. Tick Yes

2.  Next to “Name of Organisation” write WorkCover Assist
